Oxford Study Explores Sildenafil’s Potential Beyond Erectile Dysfunction: Enhancing Brain Blood Flow and Combating Vascular Dementia
Sildenafil, widely recognized by its brand name Viagra, is primarily known for its effectiveness in treating erectile dysfunction in men. However, a groundbreaking study conducted by researchers at the University of Oxford suggests that this drug may offer significant benefits beyond its current use.
According to the research, sildenafil has the potential to improve blood flow to the brain and enhance the function of blood vessels in individuals at high risk of vascular dementia.
Vascular dementia is a serious condition that significantly impacts cognitive functions, including judgment and memory. It occurs due to a reduced blood supply to the brain, which damages brain tissue. The study, published in the journal Circulation Research, represents a potentially pivotal advancement in the fight against dementia.

The researchers discovered that sildenafil can increase blood flow in both large and small brain vessels. This enhancement was measured using sophisticated techniques such as ultrasound and MRI scans.
Notably, sildenafil improved the blood flow response to carbon dioxide, indicating better cerebrovascular function. In addition to sildenafil, the study also examined cilostazol, another drug, which along with sildenafil, was found to lower blood vessel resistance in the brain. However, sildenafil was noted to cause fewer side effects compared to cilostazol, particularly having a lower incidence of diarrhea.
“This is the first trial to show that sildenafil penetrates the blood vessels in the brain of people with this condition, improving blood flow and the responsiveness of these vessels,” said Dr. Alastair Webb, Associate Professor at the Wolfson Centre for Prevention of Stroke and Dementia at Oxford University.
He emphasized that these improvements are associated with chronic damage to the small blood vessels in the brain, a common cause of vascular dementia. “This demonstrates the potential of this well-tolerated, widely-available drug to prevent dementia, which needs testing in larger trials,” Dr. Webb added.
Currently, there are no specific therapies for vascular dementia, which is caused not only by chronic damage to the small blood vessels in the brain but also contributes to 30% of strokes and 80% of brain bleeds. The OxHARP trial, conducted as part of this study, involved 75 participants who had experienced a minor stroke and exhibited signs of mild to moderate small vessel disease.
Each participant in the trial was administered sildenafil, a placebo, and cilostazol over a three-week period. To assess the effects of these drugs, the researchers utilized a combination of cardiovascular physiology tests, ultrasound, and functional MRI scans.

The results of the trial are promising, indicating that sildenafil could play a crucial role in enhancing brain blood flow and potentially preventing vascular dementia.
